Stages of cancer explained using the TNM system for nurses, nursing / healthcare students, as well as an oncology NCLEX review. Cancer Stages Quiz: 🤍 Notes: 🤍 Cancer can be graded and staged. These two terms each have different meanings and as the nurse you want to be familiar with the definition and the lingo used to communicate the grade and stage of your patient’s cancer. The tumor grades of low to high can be given numerical values to reflect this rating, but it’s important to note that some forms of cancer have they own grading systems, like breast cancer which uses the Nottingham system. Grade I: well differentiated, low grade Grade II: moderately differentiated, intermediate grade…cells have some differentiation Grade III: poorly differentiated, high grade Grade IV: undifferentiated, high grade…cells are extremely abnormal Tumor staging tells about the main tumor (location, size) and if the cancer has spread to other parts of the body like the lymph nodes or other organs. One system you want to be familiar with as the nurse is the TNM staging system. This system is used to stage cancers with solid tumors like found in colon cancer and other types. TNM stands for tumor, nodal involvement, and metastasis. The TNM system is used to calculate the stage of cancer. Brian Moore, MD, is a head and neck surgical oncologist, and he is also the director of the Ochsner Cancer Institute. Dr. Moore oversaw the expansion of the Gayle and Tom Benson Cancer Center and says the new facility allows Ochsner to not only focus on the treatment of cancer patients but also the recovery and rehabilitation that come afterward. While caring for patients with cancer can be difficult, Dr. Moore says he became an oncologist because of the long-term relationships he builds with each of his patients. The COVID-19 pandemic has undoubtedly added a layer of difficulty to cancer care, but Dr. Moore says it is because of Ochsner’s use of telemedicine that he can continue building relationships with his patients from the comfort of their homes. Michael Anderson, MD, talks about his work with cancer patients at Dana-Farber Community Cancer Care, and why four ears are better than two when it comes to first appointments. He describes the progress he's seen in cancer treatment over his career and the benefits of cancer care in the community connected to Dana-Farber in Boston. Learn more at 🤍 Transcription: My name is Michael Anderson. I am an oncologist and hematologist, and I see patients in Dana-Farber Community Cancer Care office in Weymouth. When you’re referred to us in Weymouth or any of our Dana-Farber Community Cancer Care facilities you would already have had materials sent to us by your primary care physician. We would have digested that and read and looked it over to see if we needed to do anything else in the office. We would have planned many times what we were going to do as seeing, but we still need to spend the hour or so with you in order to examine you and to ask questions ourselves. Dana-Farber Community Cancer Care is part of Dana-Farber Cancer Institute and is within that network. The majority of cases, we will be able to take care of you in the community. There are those cases that do require more specialized care. We have then the depth of resources at Dana-Farber Cancer Institute to offer to you, so the research that’s being done that’s being taken from the lab to the bedside is available to you if needed. We care very much about you and your family. We care very much about the result and the care that you receive. So, when a patient comes the first time especially, we really appreciate another family member or some significant other person being there. Four years, six years, eight years are better than just two. When you hear certain things, it will throw your mind into a certain spin that you may not want and that you may not hear the next things that we’re talking about, so having more people there really helps, and then when you get in the care and get home around the kitchen table, you can further discuss what we have already discussed. There’s a lot of information especially in diagnoses that you've never encountered before, and in that information, it takes awhile to understand it. I chose oncology as a specialty many years ago, because I knew the science of oncology would catch up to the knowledge of cancer and the meaning of cancer within my lifetime, and it’s turning out to be quite true. Now, we’re getting down to the molecular level—we’re seeing mutations, and we’re actually developing drugs that attack that mutation. So, it’s a very exciting time for oncology and the science of oncology in the fact that we can treat these mutations and find very good results.

Immuno-oncology is the study and development of certain treatments called immunotherapies for cancer. These treatments fight cancer by strengthening your immune system. Normally, the white blood cells of your immune system help fight infections and disease. They recognize substances on the surface of cells called antigens. Healthy cells have different antigens than damaged body cells or foreign invaders. Your immune cells usually ignore the antigens on healthy cells, but they do respond to antigens on damaged cells and foreign invaders. Some immune cells mark these antigens for destruction with a substance called an antibody. Then, other immune cells attack the cells that have these antibodies. Since cancer is made up of damaged cells, they may have antigens that your immune cells recognize and attack. But, some cancer cells have ways to avoid destruction by your immune cells. For example, cancer cells may reduce the number of cancer antigens, allowing them to hide from your immune cells. A second way cancer cells avoid destruction is through checkpoint proteins. Both cancer and immune cells may have these proteins. When they attach, other immune cells won't attack the cancer cell. A third way cancer cells avoid destruction happens when the cancer cells are under attack from the immune system. During the attack, the cancer cells release certain substances that call in other types of immune cells, which slow down or stop the immune response. Immunotherapies have several ways they work to help your immune system fight cancer. One way is through special antibodies made in the lab. Some of these antibodies attach to cancer cells. They can kill the cancer cell directly. Others mark the cancer cells so that your immune cells destroy it. Another group of antibodies blocks the checkpoint proteins. This keeps them from attaching to each other. As a result, the immune cell can attack and kill the cancer cell. Still other antibodies prevent the growth of cancer cells. Some of them block the signals that cause new blood vessels to grow and feed the cancer. In another type of immunotherapy, some of the immune cells that best fight cancer are removed from the body. The cells are taken to a lab where many more of them are grown. Then, they're returned to the body to attack the cancer.
