Melanoma is the most serious type of skin cancer. It develops in the cells that produce melanin, the pigment that gives skin its color. Melanoma is one of the most common cancer types in the U. S. Roughly 2% of people will be diagnosed with melanoma of the skin at some point during their lifetime, according to the National Cancer Institute. Treatment for early stage melanomas usually includes surgery to remove the melanoma. Mohs surgery is a precise surgical technique used to treat skin cancer. During Mohs surgery, thin layers of cancer-containing skin are progressively removed and examined until only cancer-free tissue remains. "Mohs surgery is essentially skin cancer removal," explains Dr. Nahid Vidal, a dermatologic surgeon at Mayo Clinic. "It's a surgical removal process that's highly specialized, where we're removing the skin cancer with a goal of not only removing all of it, but also leaving behind as much healthy tissue as possible." Mohs surgery allows surgeons to verify in real time through pathology that all cancer cells have been removed at the time of surgery. This increases the chance of a cure and reduces the need for additional treatments or additional surgery. On the Mayo Clinic Q&A podcast, Dr. Vidal discusses skin cancer and the use of Mohs surgery to treat early stage melanoma. ONE MONTH OUT FROM MOHS SURGERY MY MOHS SURGERY EXPERIENCE On December 8, 2017, I had Mohs Surgery for what my dermatologist originally told me was Basal Cell Carcinoma. Unfortunately, she did not do a biopsy at the time and I learned just days before my scheduled Mohs surgery that it was likely that I had a benign lesion instead of skin cancer. My dermatologist's failure to do the biopsy led me down a path that resulted in my having the Mohs surgery for a benign lesion. Although after the surgery I initially thought I should not have had the Mohs surgery, after talking with the surgeon, he said that underneath the skin, the tumor was more widespread then it looked on the surface and that Mohs surgery was the best way to remove the entire tumor with the best cosmetic result. I flew to Spokane, Washington to have the surgery with Dr. Conrad Chestnut as he is both a Board Certified Dermatologist and Mohs Fellow and also Board Certified in Plastic Surgery. Using generative AI, this time lapse sequence shows how squamous cell carcinoma skin cancer develops over 25 years. Starting with normal skin, sun damage eventually leads to actinic keratosis formation. In about 10% of actinic keratosis, malignant change occurs ultimately resulting in the development of skin cancer. Obviously, such a time lapse can not be realistically accomplished as there is no way to know if any given area of skin will turn into cancer. Obviously, somebody with such future knowledge would have to start taking such photos now in the same spot over next 25 years to watch it slowly turn into cancer. Horizon Medical Services Skin Cancer Treatment Expert, Dr. Mark Pomper says, "When it comes to choosing Mohs surgery or Radiation Therapy, you've got to consider the pros and cons of both treatments for your particular skin cancer situation." He breaks it all down for Health Specialist Mitzel in this interview. Mohs For Skin Cancer: Mohs is an invasive surgical treatment option to remove skin cancer. According to the Skin Cancer Foundation, it has a cure rate of 99% for skin cancer that has not been treated before and 94% for skin cancer that has recurred after prior treatment. During a Mohs procedure layers of skin are removed one by one then examined under a microscope to check for cancer cells. This is repeated layer-by-layer until they can find no more cancer cells in the last slice taken. This process is painful and can take many hours and require many slices of skin to be removed to make sure all the cancer has been removed. Expect to spend a long day at the doctor's office on the day of the procedure. Mohs is a surgical procedure, so with it come risks like bleeding, infection, and scarring. Patients most often require downtime as it takes time to heal the open wound left behind from the treatment. Radiation Therapy for Skin Cancer: Radiation is a non-surgical treatment for patients with skin cancer. It's especially helpful for difficult-to-treat areas of the body and for people older people who can't have surgery due to other medical problems, or medications they are on, or simply don't want surgery. Radiation has been used to treat skin cancer for over 100 years and also has a high cure rate. It kills the cancer by targeting the cancer directly with superficial radiation, so there is no cutting and no surgery. Dr. Pomper says, "In some cases radiation is a better than Mohs especially for an older, infirm patient. And for areas that are difficult to repair cosmetically, such as the rim of the nostril, radiation is superior because the cosmetic result is better.” According to the Journal of Clinical and Aesthetic Dermatology, superficial radiation therapy should be the first option for certain types of skin cancer because cure rates are similar to surgical options. What is the Best Type of Radiation Therapy? One of the most highly effective types of radiation to treat skin cancer is high-dose radiation (HDR). HDR therapy is a favorable option because of its effectiveness and ability to spare healthy tissue surrounding the cancer site. It is also relatively painless. It's a skin cancer remedy invented in the 1930's that's becoming more popular today. News 13's Jay Siltzer found out first hand about Mohs surgery for this special Health Alert. Dr. Michael Loosemore, "there is a sort of skin cancer epidemic you could call it going on." Under Jay Siltzer's TV makeup is a three-inch scar, "it's been 6 weeks, that's not a long time." Especially considering it resulted from cancer surgery to remove basal cell carcinoma from his forehead that looked relatively harmless. Dr. Michael Loosemore, Carolina Mountain Dermatology, "I think your case highlights the difficulty of knowing what's going on. No one would expect you to know that the spot that looks like a scar, is actually a skin cancer." Dr. Loosemore, "this is almost the nightmare scenario you have." Cancer beneath the skin 7 times larger than the initial spot, penetrating almost to the skull. It required several attempts in one day to remove the cancer, while preserving as much good tissue as possible, through Mohs surgery. "By doing the Mohs surgery we are able to look at 99 percent of the exterior margins of the tumor as well as base of tumor and know with certainty you are tumor free after the procedure." Danny Lasure, cancer survivor, "you leave here with a big sense of relief that it's done and you followed up with it, if you don't you know what the repercussions are." Danny Lasure realizes he could easily be dead from three recent bouts with skin cancer likely stemming from playing outdoors as a kid with no sunscreen. The first problem appeared on his back. Danny, "I had melanoma surgery, been about 4 years ago. After that, I had squamous cell on this side and basal cell on this side and I've had the Mohs surgery up here twice." For Danny, there is little visible sign of the procedure because of reconstructive techniques. For Jay, suturing up a nearly half-dollar-size hole proved more difficult. 20-plus stitches and several weeks later, injections are still needed to reduce swelling and blend the color, it is a scenario that's happening to more and more people from too much exposure to UV rays. Dr. Loosemore, "I mean 50, 60 year olds are absolutely the norm and unfortunately 40 year olds becoming the norm as well." Melanoma Diagnosis. Mohs Surgery and Excision for the Deadliest Skin Cancer Farm Girl went to the dermatologist for a skin exam and had regular burning of moles and spots. One mole, in particular, was alarming to the dermatologist. She took a quarter-sized piece of tissue for a biopsy and in two weeks, it came back as a precursor to melanoma. An excision surgery was scheduled and performed two weeks later. The large piece of tissue from the surgery was also sent for biopsy and in another two weeks we were informed that indeed was melanoma and that further surgery was needed. This news came on Thursday and we were told that we would get a call from another surgeon on Monday. The problem was that we were told that the surgeon that was to call us was going to open the healing wound, remove more tissue and leave the wound open overnight until a biopsy could be performed. This sounded barbaric but no other surgeons were available until Monday. When Monday came we wasted no time researching and finding the best in the area. What was crucial was finding one that performs MOHS surgery and was equipped to biopsy each piece or layer of tissue as it was removed. I was diagnosed with infiltrative basal cell carcinoma and was not prepared for the outcome. This series will share my skin cancer journey from MOHS procedure to remove it, plastic surgery to fix it, choosing a sunscreen, to how social media helped me heal. Basal carcinoma is one of the most common forms of skin cancer, and yet I had never heard of it until I was diagnosed with it. I had only ever heard of Melanoma.
