Although less common, it also can be transmitted through aerosol, meaning airborne. Most likely, when you have people in an enclosed space, such as an elevator, someone sneezes or coughs without covering their mouths, and someone else can inhale it in. This covid attaches to cells in our body by this ACE2 receptor. This ACE2 receptor is only located on specific cells in our body. It's on our tongue, in our nose, back of the throat, and in our lungs. Specifically, it's only located on our type II alveolar cells within the lungs. ARDS develops in about 4 to 5% of COVID patients. And of all the people who get covid, the mortality rate is around 1 to 2% or max 3%. So why do some COVID patients get ARDS, and why do some die? There are different reasons, and let's talk about them. It could be one of these reasons, but more likely, it's a combination of these reasons. 1. The covid only gains entry into our cells that express the ACE2 receptor. They are located on multiple sites. Besides being in the lung, they're in your mouth, nose, throat, stomach, small intestine, colon, skin, lymph nodes, thymus, bone marrow, spleen, liver, kidney, brain, and testes. 2. It makes sense that if the virus only gets into your mouth, nose, or throat, but not the lungs, it would cause only cold-like symptoms. But if the covid gets all the way down into the alveoli of your lungs, that's what's going to cause ARDS. And by the way, the ACE2 receptors in your gut probably explain why some patients get nausea, vomiting, and diarrhea. 3. The amount of virus you get into your body likely determines how sick you get. This is what we call the viral load. 4. The inflammatory reaction with COVID is extremely complicated, with many different proteins and hormones and interleukins at play. But there are several known genetic polymorphisms of these proteins that likely make some people more prone to getting worse illnesses than others. A genetic polymorphism simply means a variation on a particular gene. For example, there are genetic polymorphisms for the ACE gene and IL-6. So, basically, a lot of it just comes down to our genes. 5. Because the 5th reason has to do with estrogen. Estrogen is known to inhibit the effects of IL-6, which plays a massive role in this cytokine storm. This might explain why women have less severe disease overall than men. 6. The 6th reason is that people are already taking certain medications for those on an ACEI such as lisinopril or an ARB like losartan, telmisartan, or candesartan, or irbesartan. Or people who take hydroxychloroquine for lupus or rheumatoid disease. Or people who take tocilizumab, an IL-6 receptor inhibitor. Are these COVID patients less prone to getting severe illnesses? Most infected people, around 80 percent, will have mild symptoms which disappear after a few days. But coronaviruses can multiply fast... spreading from the throat to the lungs. And that's how they can infect lung cells until pneumonia sets in. The body is supplied by oxygen via the lungs - entering the bloodstream via the alveoli to the blood vessels. And then it is distributed to the rest of the body. Complications can occur when the coronaviruses settle in the space between the pulmonary alveoli and the blood vessels. That can cause inflammation. The distance to the blood vessels then becomes greater and oxygen intake more difficult. When organs are deprived of oxygene, breathing is labored. And then respiratory muscles are quickly exhausted, especially in the elderly. That's when the lungs need help - with a ventilator. But that can't prevent the situation from worsening in some cases. Inflammation in the lungs increasingly hinders gas exchange. Water escapes from the cells. Even with the support, not enough oxygen enters the body. Then an external machine must take over the function of the lungs. The so-called "Emco" enriches the blood outside the body with oxygen and then returns it into the body - but not without complications. But the situation can escalate even further.The immune system then becomes the main problem - triggering a massive inflammation as a reaction. Progression to severe cases now less common than during the first wave of the coronavirus pandemic. This could be due to mask usage...it may contribute to a smaller amount of the infection in the air - making the disease less deadly. Defensive compounds flood the entire body. Immune cells attack the inner walls of blood vessels. They become weak. Fluid leaks into the tissue and is deficient in the cirulatory system. The organs are no longer sufficiently supplied with blood. But most COVID cases are mild. The recuperating phase can cause some concentration problems, exhaustion, and muscular weakness. But this is not more severe than other infections. It could be that the virus moves through the nervous system of people it infects. That's what new research suggests. Hey guys! Dr Sharma DO here! The deadly strain of virus that has been causing severe respiratory infections can cause low oxygen levels and damage to our oxygen exchange system, known as alveoli. The damage can be so severe that membrane damage results in protein leakage into our lungs - this causes a 'hyaline membrane' which makes gas diffusion even more difficult. This is called acute respiratory distress syndrome, and is officially calculated using the gradient of alveolar oxygen (predicted) versus how much oxygen is actually in the blood via an ABG (arterial blood gas). Essentially patients will require positive pressure ventilation or even mechanical ventilation to pressurize oxygen diffusion into their blood stream. This assistance has a limit, however, and at a certain point, oxygen levels just cannot be maintained to keep the patient alive, resulting in other adverse medical situations such as cardiac arrest/cardiac arrhythmia and hypoxemic brain damage. The number of people killed by the new coronavirus rose by 97 on Sunday, the highest number of casualties in a day. The total number of deaths in China is now 908 - but the number of newly-infected people per day has stabilised. Across China, 40,171 people are infected while 187,518 are under medical observation. The virus isn’t the killer—it’s the immune system. Here’s what you need to know. It’s not the virus that kills you. It’s actually your own immune system. The virus doesn’t have a metabolism. It’s not even alive. Here’s what happens. You have this inactive shell with some DNA that has some very specific instructions to replicate. There are certain things on the surface of the virus that connect to certain tissues and certain receptors in the cell. For example, COVID-19 connects to the ACE2 receptor. Once it connects, it’s able to get into the cell. Once inside of the cell, the immune system doesn’t see it at first. The goal of the virus is to get into the cell and replicate as fast as possible. This creates a lot of damage and an immune reaction, but it’s not what kills you. Let’s say you have COVID-19, you’re on a respirator, and you have cytokine storm. This is creating massive destruction, which can lead to death. This destructive inflammatory condition is also happening on the inside of your arteries, and it can even create blood clots. The innate part of the immune system is overreacting because part of the adaptive immune system is not working. The T suppressor cells, which are a part of the adaptive immune system, suppress or regulate the overreaction. But, there is a severe imbalance going on where you lose the adaptive immune system, including the T suppressor cells, and there is too much of the innate immune system causing the overreaction. This is what’s really killing people. The virus is a trigger, and it causes damage, but the virus mainly seems to kill people with a weak immune system. It’s usually certain metabolic conditions that cause weakness within the T suppressor cells. If your immune system is strong, the coronavirus does not seem to be a big threat. On Monday evening, Boris Johnson took the unprecedented step of curtailing individual freedoms to prevent the further spread of coronavirus.
