RBC also known as erythrocytes are produced in the bone marrow and then enter circulation to become mature. The mechanism of RBC haemolysis can be intravascular which means destruction of RBC within the vasculature or haemolysis can occur extravascularly. Which means haemolysis outside the vasculature typically in organs such as the spleen and liver where the reticuloendothelial system resides. Haemolysis stimulates the kidneys to produce EPO a hormone which stimulate erythropoesis the production of RBC. RBC are produced via the myeloid progenitor cell line to become erythrocytes in the bone marrow. Erythropoesis is also stimulated by other hormones including androgens and thyroid hormone." The ability of bacterial colonies to induce hemolysis when grown on blood agar is used to classify certain microorganisms, particularly for streptococcal species. (Types) • α-hemolysis • β-hemolysis: CAMP test • γ-hemolysis (Comparison) • hemolysis: Process by which a bacteria use hemolysins (substances that cause hemolysis) to specifically permeabilize and lyse blood cells. • hemodigestion: Nonspecific killing of blood cells by metabolic by-products of bacteria. α-hemolysis (green hemolysis, incomplete hemolysis, partial hemolysis): Dark and greenish. Green oxidized derivative methemoglobin caused by hydrogen peroxide produced by the bacteria. (Example of bacteria) • Streptococcus pneumoniae • Streptococcus mutans • Streptococcus viridans hemedigestion: It turns green in confluent growth, whereas, no change around single colonies. (Example of bacteria) • Vibrio cholerae β-hemolysis (complete hemolysis): Lightened (yellow) and transparent. Complete lysis of red blood cells in the media caused by streptolysin, an exotoxin, enzyme produced by bacteria. • Streptolysin O: Oxygen-sensitive cytotoxin, secreted by most Group A streptococcus and Streptococcus dysgalactiae, and interacts with cholesterol in the membrane of eukaryotic cells (mainly red and white blood cells, macrophages, and platelets). • Streptolysin S: Oxygen-stable cytotoxin also produced by most Group A streptococcus strains. Affects immune cells, including polymorphonuclear leukocytes (neutrophils, eosinophils, basophils, and mast cells) and lymphocytes, and is thought to prevent the host immune system from clearing infection. CAMP test: Some weakly beta-hemolytic species cause intense β-hemolysis when grown together with a strain of Staphylococcus. • Streptococcus agalactiae • Clostridium perfringens • Listeria monocytogenes γ-hemolysis (non-hemolytic): Color unchanged. It is a form of low hemoglobin due to the destruction of red blood cells, increased hemoglobin catabolism, decreased levels of hemoglobin, and an increase in efforts of bone marrow to regenerate products. There are many types of hemolytic anemia. The condition can be inherited or acquired. "Inherited" means your parents passed the gene for the condition on to you. "Acquired" means you aren't born with the condition, but you develop it. Hemolytic anemia can occur due to erythrocyte or extra-erythrocyte causes. Specifically: Extra-erythrocytic disorders: Causes of extra-erythrocytic disorders that patients with hemolytic anemia often encounter, including: Increased activation of the interretinal endothelial system: hypersplenism. Immune abnormalities: neonatal hemolytic anemia due to blood group incompatibility, autoimmune anemia, thrombocytopenic purpura. Mechanical trauma Some drugs cause hemolysis: quinine, quinidine, penicillin, methyldopa, clopidogrel. Poisoning: lead, copper... Infection: pathogenic bacteria can cause hemolysis through the direct action of bacterial toxins (clostridium pefringens, alpha-beta hemolytic streptococci, meningococcal disease), caused by bacteria that invade and destroy red blood cells (plasmodium sp...) or by producing antibodies (Epstein Bar, mycoplasma). RBC abnormalities: Intrinsic defects of red blood cells can cause hemolysis involving abnormalities of cell membranes, cell metabolism, or hemoglobin structure. RBC membrane abnormalities: minkowski chauffard disease (reduced erythrocyte strength, autosomal dominant inheritance), squamous cell disease (beta lipoprotein abnormalities), rhabdomyolysis (dominant inheritance) , paroxysmal nocturnal hemoglobinuria (acquired erythrocytosis). Chapters 0:00 Introduction 1:26 Causes of hemolytic anemia 2:30 Symptoms of hemolytic anemia Diagnosis of hemolytic anemia 3:03 Treatment of hemolytic anemia Hemolytic anemia is a form of anemia due to hemolysis, the abnormal breakdown of red blood cells (RBCs), either in the blood vessels (intravascular hemolysis) or elsewhere in the human body (extravascular).[1] This most commonly occurs within the spleen, but also can occur in the reticuloendothelial system or mechanically (prosthetic valve damage).[1] Hemolytic anemia accounts for 5% of all existing anemias.[1] It has numerous possible consequences, ranging from general symptoms to life-threatening systemic effects.[1] The general classification of hemolytic anemia is either intrinsic or extrinsic.[2] Treatment depends on the type and cause of the hemolytic anemia.[1] Symptoms of hemolytic anemia are similar to other forms of anemia (fatigue and shortness of breath), but in addition, the breakdown of red cells leads to jaundice and increases the risk of particular long-term complications, such as gallstones[3] and pulmonary hypertension.[4] Symptoms of hemolytic anemia are similar to the general signs of anemia.[1] General signs and symptoms include: fatigue, pallor, shortness of breath, and tachycardia.[1] In small children, failure to thrive may occur in any form of anemia.[5][6] In addition, symptoms related to hemolysis may be present such as chills, jaundice, dark urine, and an enlarged spleen.[1] Certain aspects of the medical history can suggest a cause for hemolysis, such as drugs, medication side effects, autoimmune disorders, blood transfusion reactions, the presence of prosthetic heart valve, or other medical illness.[1] Chronic hemolysis leads to an increased excretion of bilirubin into the biliary tract, which in turn may lead to gallstones.[7] The continuous release of free hemoglobin has been linked with the development of pulmonary hypertension (increased pressure over the pulmonary artery); this, in turn, leads to episodes of syncope (fainting), chest pain, and progressive breathlessness.[8] Pulmonary hypertension eventually causes right ventricular heart failure, the symptoms of which are peripheral edema (fluid accumulation in the skin of the legs) and ascites (fluid accumulation in the abdominal cavity).[8]

PNH is a rare and lethal disorder of the blood. Read key information on the origin and treatment of the disorder in our latest collection and animation: 🤍 Paroxysmal nocturnal hemoglobinuria (PNH) is a rare, acquired, life-threatening disease of the blood characterized by destruction of red blood cells (known as hemolysis) by the complement system, a part of the body's innate immune system. Tell-tale signs and symptoms of the disorder include fatigue, shortness of breath, abdominal pain, sudden blood clotting, or thrombosis, and—as the disorder’s name suggests—dark, hemoglobin-rich urine. Current treatment methods involve either bone marrow transplantation or treatment with complement protein inhibitors such as eculizumab, which reduce mortality, improve quality of life and reduce need for transfusions, however the search for a definitive cure continues. Lesson on Autoimmune Hemolytic Anemia (Warm, Cold, and Paroxysmal Cold Hemoglobinuria): Clinical features, Laboratory findings, Signs and Symptoms, Diagnosis, and Treatment. Autoimmune Hemolytic Anemia is one type of anaemia that is due to the production of autoantibodies against red blood cell membrane antigens. Each subtype of autoimmune haemolytic anaemia has a different type of autoantibody that is produced. Each of the subtypes of autoimmune haemolytic anaemia have different categories of causes, which we discuss in this lesson. These different types of autoimmune haemolytic anaemia have many clinical features in common including symptoms of anemia, and signs and symptoms of haemolysis. Catherine Broome, MD, Lombardi Cancer Center, Washington, DC, gives an update on autoimmune hemolytic anemias. Warm autoimmune hemolytic anemia accounts for around 80% of all autoimmune hemolytic anemias, and it’s mediated by an IgG autoantibody. While cold autoimmune hemolytic anemia is less common, accounting for about 15-18% of cases, and it’s mediated by an IgM autoantibody that subsequently, after binding to the antigen on the surface of the red blood cell, activates the classical pathway of complement, which is responsible for hemolysis. This interview took place at the Lymphoma, Leukemia & Myeloma Congress 2021.

Anti-A and anti-B antibodies - most of the time - are IgM (i.e. they don't cross the placenta, but some mothers, naturally, have IgG anti-A or IgG anti-B antibodies, (i.e. can cross the placenta causing problems to the fetal RBCs.) This most commonly occurs within the spleen, but also can occur in the reticuloendothelial system or mechanically (prosthetic valve damage). Hemolytic anemia accounts for 5% of all existing anemias. It has numerous possible consequences, ranging from general symptoms to life-threatening systemic effects. The general classification of hemolytic anemia is either intrinsic or extrinsic. Treatment depends on the type and cause of the hemolytic anemia. Symptoms of hemolytic anemia are similar to other forms of anemia (fatigue and shortness of breath), but in addition, the breakdown of red cells leads to jaundice and increases the risk of particular long-term complications, such as gallstones and pulmonary hypertension. Symptoms of hemolytic anemia are similar to the general signs of anemia. General signs and symptoms include: fatigue, pallor, shortness of breath, and tachycardia. In small children, failure to thrive may occur in any form of anemia. In addition, symptoms related to hemolysis may be present such as chills, jaundice, dark urine, and an enlarged spleen. Certain aspects of the medical history can suggest a cause for hemolysis, such as drugs, medication side effects, autoimmune disorders, blood transfusion reactions, the presence of prosthetic heart valve, or other medical illness. Chronic hemolysis leads to an increased excretion of bilirubin into the biliary tract, which in turn may lead to gallstones. The continuous release of free hemoglobin has been linked with the development of pulmonary hypertension (increased pressure over the pulmonary artery); this, in turn, leads to episodes of syncope (fainting), chest pain, and progressive breathlessness. How to inoculate bacteria on blood agar plate: • Grow bacteria overnight (use a fresh inoculum) • Inoculate blood agar plate using your favorite streaking method • “After overnight growth 37 C • Held up to a light source light coming from behind 2. Beta Hemolysis • Complete or true lysis (A clear zone) • Toxic by-products • Destroy red blood cells • Multiple toxins !!! • Beta hemolytic Streptococcus species, Streptococcus pyogenes • Hemolysin that is oxygen labile Streptolysin O (low oxygen level) • Anaerobic environment by inserting the inoculating loop vertically in the media • (Stabbing) • Oxygen stable hemolysin Streptolysin S (normal oxygen level) 3.
