Центр имплантации All-on-4: 🤍 Важные вопросы об Имплантация All on 4 и All-on-6. 🤍 Врач, стоматолог-ортопед, имплантолог, хирург, Якубов Алексей Исаевич рассказывает подробности о методике имплантации челюсти при полном отсутствии зубов "Все на четырех". Методика All-on-4 получила широкое распространение во всем мире. Она признана как эффективная методика ведущими стоматологическими школами всего мира. Изначальна она была разработана для тех ситуаций, когда у человека нет достаточно кости, чтобы сделать классическое протезирование на имплантатах. Это несъемное протезирование на всю челюсть опирающееся на 4 имплантах, это суть методики. Фиксация имплантов на верхней челюсти не такая сильная как на нижней челюсти, поэтому мы предпочитаем устанавливать на верхней челючти 5 или 6 имплантов и это гарантирует нам результат. Для чего проводится компьютерная томография? Компьютерная томограмма - 3d исследование, позволяет нам оценить состояние кости, толщину кости, спрогнозировать, спланировать места установки имплантов, размеры имплантатов, и соответственно понимать вообще возможно ли изготовление такой конструкции. Это очень важный диагностический метод. Он в принципе очень важный, а для All-on-4 он критически важный. Какие виды обезболивания применяются? Все делается под местной анестезией. Достаточно мощные анестетики, поэтому во время операции пациент ничего не чувствует. В определенном небольшом проценте случаев мы используем седацию для пациентов у которых есть фобии, которые боятся стоматологов. Общий наркоз используется крайне редко. Обычные уколы, чуть-чуть чувствительные. Во время самой операции нет никакой чувствительности. Плюс ко всему, если операция немного затянулась и анестезия потихонечку отходит у нас есть всегда возможность добавить в процессе анестезию, поэтому боли никакой нет во время операции. Как проходит приживление имплантатов? Как долго прослужит несъемный протез? Если правильно использовать хирургический и ортопедический протокол, а мы стараемся так и делать, то статистика приживляемости она такая же как при обычной имплантацию у нас, она составляет 99.5 процентов, и ничем не отличается от классической имплантации. Человек фактически на 1-7 день, т.е. от одного до семи дней изготавливается конструкция, он получает уже несъемные зубы и может нормально функционировать, и улучшается качество жизни. Поэтому 1-7 дней это тот срок при котором мы фиксируем несъемные зубы. Срок службы конструкции All-on-4 по статистике, по статистическим исследованиям он считается более 10 лет. Если пациент соблюдает все рекомендации и периодически посещает на осмотры своего врача стоматолога, то есть все предпосылки и все возможности для того, чтобы эта конструкция служила более 10, более 15 лет, и даже более 20 лет, так как при обычной классической имплантации срок службы более 20 лет это для нас уже рутина. Поэтому я считаю, что конструкция All-on-4, при правильной эксплуатации и соблюдении всех рекомендаций может спокойно прослужить более 20 лет. Из клинической практики. Пациентка обратилась с такой ситуацией (указывает на рентгеновский снимок). Полное отсутствие зубов на нижней челюсти, и частичное отсутствие зубов на верхней челюсти, старая металлокерамическая конструкция со съемным протезом. Зубки с воспалениями, вторичным кариесом, разрушенные корни, которые подлежат удалению. На нижней челюсти у нас в боковых отделах атрофия кости, и невозможность установки классических имплантов, и достаточно хорошая, мощная кость в переднем отделе от 5 до 5 между ментальными отверстиями. На верхней челюсти мы видим верхнечелюстные гайморовые пазухи и отсутствие полное кости в боковом отделе и справа и слева. И достаточно неплохая кость в переднем отделе. У этой пациентки было спланировано удалению оставшихся зубов на верхней челюсти, одномоментная установка 6 имплантов. Два импланта в боковом отделе в обход синуса и 4 импланта в переднем отделе в области передних зубов. На нижней челюсти пациентка изъявила желание сделать не на 4, а на 5 имплантах, финансы ей позволяли, поэтому мы спланировали установку 5 имплантов, два импланта в обход ментальных отверстий, в обход нерва в боковых отделах, и три импланта в переднем отделе. Дальше хочу показать рентгеновский снимок сразу после операции. Мы видим на верхней челюсти, чтобы не попасть верзнечелюстные синусы были установлены два импланта под углом, которые дали нам опору боковых зубов. 4 импланта в переднем отделе. На нижней челюсти были установлены два угловых импланта в обход нерва, и три импланта в переднем отделе. Этой пациентке был сразу снят слепок и в течение 7 дней была изготовлена несъемная конструкция. Это вид пациентки через неделю уже с установленными зубами.

All on six has advantages over All on four as more strength and stability are provided for your jawbone with the additional implants. All on 6 dentures can last up to 20 years. What is an ALL-on-6? The ALL-on-6 dental implant procedure is the creation of a complete dental prosthesis, anchored to the jawbone (top) or mandible (bottom) by six titanium implants, integrated into the gum and evenly distributed along the jaw. An arch, topped with false gums and ceramic or porcelain dentures, completes the device by screwing onto the six implants embedded in the jaw. This orthodontic treatment, which has the advantage of being able to be implanted even on a low volume of jawbone, is intended for patients whose mouths are completely or partially edentulous in one or two jaws, or who have advanced deterioration of many teeth. It corrects a bite defect, restoring more harmonious proportions to the lower face. The ALL-on-6 procedure is usually performed in two sessions about four months apart while the implants integrate perfectly into the bone and gums. During this time, a temporary prosthesis is attached to the implants. The ALL-on-6 solution avoids the more invasive bone grafting and offers a faster recovery time and lower cost than other implant treatment techniques. In addition, the creation of a bridge with a false gum overlay does not require post-implant surgery like a sinus lift. If the jawbone is too thin, too weak or degraded, a bone graft will be prescribed. Usually created with synthetic biocompatible material, it reinforces the bone on which the implant will be implemented. Unlike dentures, ALL-on-6 treatment is fixed and permanent : it does not require the use of adhesives, it does not move and it does not deteriorate over the years. If a traditional dental implant is used to replace isolated damaged teeth, this system is aimed at replacing a complete set of teeth on the upper and/or lower jaw. Most importantly, it avoids the deterioration of the jawbone that occurs when there are no natural or artificial tooth roots. In addition to a natural aesthetic appearance, ALL-on-6 dental implants also provide permanent, efficient and long-lasting chewing, allowing patients to enjoy life to the fullest.
